Trouble Falling Asleep: What Is Known and What May Help

What short-term insomnia is
Insomnia is difficulty falling asleep, staying asleep, or waking too early, with daytime consequences. Short-term insomnia is common and usually tied to stress, travel, shift work, caffeine, alcohol or a short illness.
Sleep is driven by a daily rhythm and by sleep pressure that builds through the day; light, timing and daytime activity all feed into it.
What mainstream guidance suggests
The NHS and Mayo Clinic both put sleep hygiene first: a regular wake time, a dark and cool bedroom, cutting caffeine after midday, limiting alcohol, and getting out of bed if you are awake for long. Cognitive behavioural therapy for insomnia (CBT-I) is the first-line approach for ongoing insomnia and is more effective than a tablet over the long term.
Where a tablet is prescribed, it is usually short-term. Melatonin is used for some circadian problems under advice, and NICE notes that the evidence for most supplements marketed for sleep is weak.
What is not established
Magnesium, valerian and most other over-the-counter sleep ingredients have not been established as effective in good trials. NIH's complementary-health centre reviewed the magnesium research and found very little rigorous evidence either way.
Self-care that is generally suggested
- Keep a fixed wake time, including at weekends.
- Make the room dark, quiet and cool.
- Cut caffeine after midday and go easy on alcohol at night.
- Get daylight and activity earlier in the day.
- If you are awake for a long stretch, get up and do something quiet until sleepy.
- Ask about CBT-I if poor sleep continues for weeks.
When to seek care
See a clinician if poor sleep lasts more than a few weeks, or if it is joined by low mood, falling asleep during the day, loud snoring with pauses in breathing, or restless legs. Do not drive when dangerously sleepy. Discuss any sleep medicine, including supplements, if you are pregnant, breastfeeding, or taking other medicines, and never give a sleep product to a child under two without advice.
