What is known

The common kinds

A tension-type headache is a dull pressing pain on both sides, often felt as a band around the head. It is the most common kind and is linked with stress, tiredness, dehydration, missed meals and screen time.

Migraine is different: usually one-sided, throbbing, worse with movement, and often with nausea or light and sound sensitivity. Sinus and medication-overuse headaches are two more patterns a clinician can separate.

What mainstream guidance suggests

The NHS suggests resting, drinking water, and simple pain relief taken as directed and not too often; using painkillers on more than about ten days a month can itself cause headaches. Noting what came before each headache is the usual way to find a trigger.

Keeping regular sleep, meals and fluids is the first-line advice for recurrent tension headaches.

What is not established

Very few shelf products have good evidence for an ordinary headache. Magnesium and riboflavin are studied mainly in migraine; for tension headaches the evidence is thin, and hydration and routine come first.

Self-care that is generally suggested

  • Water and a proper meal, especially if you have skipped one.
  • Rest in a quiet, dim room.
  • Paracetamol or ibuprofen as directed, and not on too many days.
  • Regular sleep and fewer long screen stretches.
  • Keep a short diary of what precedes each headache.
  • See a clinician if headaches are getting more frequent.

When to seek care

Get urgent help for a sudden severe headache unlike any before (a thunderclap), a headache with fever and a stiff neck, confusion, weakness or slurred speech on one side, a head injury, or a headache after exercise that comes on explosively. See a clinician if headaches are new after age fifty, getting worse, waking you at night, or coming more than a few days a month. Pregnancy or a weakened immune system changes the advice, and a child under two with a headache should be seen.
