What congestion actually is

The nose has a rich blood supply, and the lining swells when it is inflamed by a cold, an allergy or dry air. That swelling narrows the airway, which is why the nose feels blocked even when there is little mucus.

The cycle naturally shifts from one nostril to the other, and it worsens when you lie flat.

What mainstream guidance suggests

The NHS and Mayo Clinic both suggest saline rinses or sprays as a first step, because salt water thins mucus and moistens the lining. Decongestant sprays are effective but should not be used for more than about seven days, since longer use causes rebound congestion; decongestant tablets are avoided by people with high blood pressure and in some other conditions.

A cool-mist humidifier and plenty of fluids are the other common suggestions.

What is not established

No spray or tablet is established as shortening the underlying cold or allergy. Congestion products work on the swelling and the mucus, which is a comfort effect.

Self-care that is generally suggested

  • Saline spray or a saline rinse, as often as the pack allows.
  • Fluids through the day and a cool-mist humidifier at night.
  • A short course only of a decongestant spray - not beyond about seven days.
  • Sleep with your head slightly raised.
  • Avoid smoke and other irritants.
  • For allergies, reducing pollen or dust exposure is the first step.

When to seek care

Seek urgent help if congestion comes with severe facial pain or swelling around an eye, a stiff neck, confusion, or difficulty breathing. See a clinician if congestion lasts more than ten days without improving, keeps worsening after a week, is one-sided with bleeding, or comes with fever above 38 degrees Celsius (100.4 degrees Fahrenheit). Mention pregnancy, high blood pressure or a weakened immune system, and take a child under two to a clinician.
